Mai: Hi, I need a document that shows I have a vaccine. It is for a new job.

Sharon: You need a vaccination record — which vaccine? Hepatitis B, or something else?

Mai: Hepatitis B, and flu, maybe.

Sharon: Do you remember when you got them here?

Mai: Last year, this hospital.

Sharon: Okay, I can print your record from our system. Can I see your ID?

Mai: Here.

Sharon: Here you are — this shows both vaccines with the dates. Your company should accept this.

Mai: If they need more, like an official stamp?

Sharon: Then go to the Health Promotion counter, Room 110, they can add an official stamp.

Mai: Okay, thank you, Sharon.
